I enjoy working at West Fork Services because I receive experience and enjoy my co-workers and the staff is helpful when I have a question or concern related to work. My supported community living staff has helped me with cooking and that has enabled me to make recipes independently. My staff has also helped me with financial concerns and other issues I have experienced that I may not have been able to resolve independently.

HCBS Day Habilitation Services: Services that assist or support the consumer in developing or maintaining life skills and community integration. Services must enable the consumer’s intellectual functioning, physical and emotional health and development, language and communication development, cognitive functioning, socialization and community integration, functional skill development, behavior management, responsibility and self-direction, daily living activities, self-advocacy skills, or mobility.
Prevocational Services: Services that prepare a consumer for paid or unpaid employment. It includes teaching the consumer job readiness skills that may include the following: following directions, attending to tasks, task completion, problem solving, and safety and mobility training.
Respite: Respite care services are provided to the consumer that gives temporary relief to the usual caregiver and provides all the necessary care that the usual caregiver would provide during that time period. The purpose of respite care is to enable the consumer to remain in the consumer’s current living situation.
Home and Vehicle Modifications (HVM): Physical modifications to the home and/or vehicle that directly address the consumers medical health or must be necessary to provide for the medical needs, welfare and safety of the consumer and to increase or maintain independence.
Supported Community Living (SCL): SCL provides one to twenty-four hours of support per day based on the consumer’s needs. This service is designed to assist the consumer with daily living needs. Assistance may include, but is not limited to: personal and home skills, community skills, personal needs, transportation and treatment services.
Supported Employment (SE): Individualized services associated with obtaining and maintaining competitive paid employment for individuals, because of disability, require intense and ongoing support to perform in a work setting. The two components of this service are to obtain a job, and supports to maintain employment.
Job Development: A Job Developer assists individuals with the help of Vocational Rehabilitation, in finding jobs within the community.
Transportation: Transportation services for consumers to conduct business errands, essential shopping, to receive medical services, to travel to and from work or day programs, and to reduce social isolation.
Supported Community Living: Provides 24 hour daily support based on the consumer’s needs.
