Patrick T. Tate, Chief Executive Officer
Patrick Tate was born and raised in Northwest Iowa, in Webster City. Patrick graduated from Webster City High School in 1971, graduated with his AA from Iowa Central Community College in 1973, and graduated with his BS in Education from Iowa State University in 1976. He holds a Certification in Rehabilitation Facility Administration from University of Missouri at Columbia in 1987. Patrick is also a 2005 graduate of the Iowa Association of Community Providers Leadership Institute.
For the past 32 years, Patrick has been a Community Provider, providing Work Services and Residential Services to people with Intellectual Disabilities. Patrick has spent the last 26 years in an administrative capacity.
Patrick is currently serving his second term on the Board of Directors for the Iowa Association of Community Providers and serves on the Governmental Relations Committee.

Joan Kellner, Chief Financial Officer/Assistant CEO
Joan started her duties at West Fork Service, Inc. in May of 1999. Her primary duties include oversight of the Business Office including budget preparation, monthly financial reports, and Audits of financial records by regulatory auditors of County, State, and Federal government. Prior work experience includes serve with USDA Farm Service Agency as Price Support Clerk, and Pharmacists Mutual Insurance as Accounting Clerk.
Joan is a 2002 graduate of Iowa Central Community College. She is a member of National Honor Society, and Phi Theta Kappa. Joan is also a member of Business and Professional Women (BPW) of Humboldt, and Humboldt Mental Health Coalition. Joan’s interest includes history, writing, and music.

Sonya Middleton, Vocational Director/Case Coordinator
I am the Director of Vocational and Case Coordination here at West Fork services. I started in this field of Human services in Aug. 1990 in California. I have had many different hats in my career; I started off as a job coach in California and after that I have had these hats throughout, Assistant Director, Director, Home Manager, Vocational Director, Community Integration Coordinator, Case Coordinator, and now my current position. I love to work with people that make me happy and when they are happy because they have accomplished a simple task. I have worked with Special Olympics in many different sports. I was an Iowa National Special Olympics coach for Track and Field. I am also an Advisor for the Kiwanis Aktion Club, which is a service club that is sponsored by the Kiwanis and we have several fund raisers and raise money for many different project in our community. I don’t know what I would be doing if I was not working with people with intellectual disabilities. I love my job!
